I hereby authorize the staff at Keepermax Enterprise LLC Camp, (at Keepermax Enterprise LLC) to act according to their best judgement in an emergency requiring medical attention, and hereby waive and release the Keepermax Enterprise LLC, along with their Camps, Clinics and its staff from any and all liability for any injuries incurred while at camp.Liability Release StatementI, the parent / guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the KeepeMax Enterprise LLC Camps or Clinics, and its affiliated organizations and sponsors. Recognizing the possibility of physical injury, associated with soccer and in consideration for the KeepeMax Enterprise LLC Camps and Clinics accepting the registrant for its soccer programs and activities, I hereby release, discharge and/or otherwise indemnify KeeperMax Enterprise LLC Camps and Clinics, all Board members, coaches, its affiliates, organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities used for the programs, against any claims by or on behalf of the registrants as a result of the registrant’s participation in the programs. Further, participation in summer camp programs could increase your risk of contracting COVID-19. READ CAREFULLY BEFORE SIGNING * By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
